Alumni Chapter Officer Report (Form L)

This report is essential, as it is the only way the Central Office
staff and volunteer leaders have to effectively communicate with alumni chapter leaders.

Alumni Chapter Name Change Request Form
|
|
For alumni chapters wishing to change the official name of the alumni
chapter. After the initial naming of an alumni chapter, a name change
for an alumni chapter in good standing can only take place in odd years
at the time of annual refranchising, which occurs from April 1 - June 30
for the upcoming fiscal year.

Annual Chapter Census
|
|
Collegiate chapters must complete and submit this Annual Chapter Census each year between February 1
and March 15. Once submitted, a copy is stored in the chapter’s Document Well and is used to assist
chapter members, volunteer leadership and staff with accessing the membership, finances, property and
management of the chapter.

Membership in Delta Sigma Pi is considered a lifetime commitment, however extreme circumstances
may arise that warrant a member requesting withdrawal. If approved
by the National Board of Directors, this is a permanent removal from membership with no
eligibility for reinstatement.
